You should carefully read the instructions and explanations before completing the form. Why? Please, pay attention to terms and conditions for winding data calculation, this time especially to those related to motor cooling. An excerpt is here:

The calculation will be performed for the air-cooled (TEFC, ODP), three-phase motors for continuous duty cycle S1, standard efficiency, 50 or 60 Hz, SF =1.00, lap or concentric, single or double-layer, random wound winding for the maximum power from the given iron core. Note that standard calculation is performed for TEFC ( Termaly Enclosed Fan Cooled ) motors ( see the photo below).

If your motor has a different type of cooling (TENV, water or oil cooling, encapsulated winding etc), such a information must be provided in advance ( see the motor with no fan).

Attaching a some photos is always useful.
